To all prospective members of the 2012-13 UVM MIHC, please note that UVM ID, full equipment & insurance info / waivers are required for all ice sessions. These ice times are only open to active UVM students.
Required: UVM ID, Insurance Info, & Full Equipment.

Here is what you need to know &/or do:
One must be enrolled at the Univeristy of Vermont. Undergraduates must be taking 9 credit hours, while graduate students must be taking 6 credit hours.
One must have a cumulative college grade point average of 2.0 or greater.
One must be aware of the team's tryout dates held in the fall and listed on our tryouts page & above.
Required Paperwork
Tryout Questionnaire - This document must be completed and submitted to the coaching staff in order to be considered for a position on this team.
UVM Club Sports Participation Waiver - Every student must complete this form and hand it to the coaching staff prior to stepping on the ice.
**NOTE**
All applicants should download the questionnaire and waiver from this page.
Please print, complete and return questionnaire to the coaching staff or the President of the team at your earliest convenience.
Other Tryout Necessities
A Tryout Identification Tag: This number tag is to be worn during every tryout session and is issued to each candidate by the coach when their picture is taken and their paperwork is received.
A Digital Headshot (with Tryout Identification Number Tag). This picture must be taken during the tryout to be considered.
